Findings: Alleged Victim (AV) relies on the facility for transportation. On or about July 14, 2022, when AV was returning from an appointment to the facility, AV was sitting in AV’s wheelchair 3-5 feet in the air and fell off the lift hitting the cement and receiving a head injury and was sent to the hospital. The facility failed to assure resident was safe, which is a violation of resident rights, is neglect of care and constitutes abuse.
