Findings: The Alleged Victim relies on the facility for his or her safety. On or about July 11, 2022, around 7:30pm the facility was notified, AV had been found attempting to hitchhike by a convenience store. It is believed the lobby doors were not locked. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
