Findings: Witness #1 (W1) is known to have behaviors and has been involved in multiple altercations. No interventions were put into place to decrease W1's altercations. On or about July 12, 2022, W1 grabbed the Alleged Victim (AV) when he/she was trying to get off the couch. W1's behavior caused AV to sustain a scrape and a couple bruises. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
