Findings: The Alleged Victim (AV) relies on the facility for their care, and is care planned to use a walker for ambulation. According to an investigation, or about July 19, 2022, facility staff got AV up for breakfast and discovered AV couldn't walk. AV's ankle was bruised and swollen, so AV was given pain medication, put in a wheelchair, and no further assessment was completed. Hours later AV was screaming in pain when staff were attempting a toileting transfer and was transported to the hospital and diagnosed with a dislocated and broken ankle which required surgery. The facility failed to provide services and timely medication treatment, resulting in unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse
