Findings: The Alleged Victim (AV) moved into the facility on July 12, 2022, and relies on the facility for their care. According to an investigation, over approximately the next 7 days, AV was left for an extended time period in a wet brief, developed a rash on their bottom, and was ultimately sent to the hospital on July 19, 2022 and diagnosed with a UTI and severe dehydration. The facility failed to provide services which is a violation of resident rights, is considered neglect of care and constitutes abuse.
