Findings: The Alleged Victim (AV) was known to have a history of altercations and wandering into other residents rooms. AV's service plan did not have any interventions in place regarding wandering. On or about July 18, 2022, AV wandered into Witness #2's (W2) room and got into an altercation with Witness #1 (W1) and W2 where the two residents slapped AV. The facility failed to properly care plan, placing AV at risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
