Findings: According to the documentation, Alleged Perpetrator (AP2) failed to follow the Alleged Vicitm’s (AV) care plan by providing 30 minute checks during the night for incontinence. The failure resulted in the AV to be found in clothes soaked in urine and feces for an undetermined amount of time, which is a violation of resident rights and is considered abuse. The facility failed to follow the AV’s care plan which is a violation of Oregon Administrative rules.
