Findings: The Alleged Victim (AV) is known to have physical and verbal behaviors towards resident and staff and is care planned to give space and re-approach when AV is exhibiting behaviors. On or about July19, 2022, AV was overheard screaming by other staff for Alleged Perpetrator #2 (AP2) to "let me go" and "get out". When staff entered AV's room to assist, they observed AP2 roughly gripping AV's arms. Staff noticed a skin tear to AV's arm after the incident. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
