Findings: The Alleged Victim (AV) is care planned to receive three stand-by assist showers weekly. According to an investigation, the facility failed to follow AV's care plan on multiple occasions regarding showers from approximately May 27, 2022, and July 15, 2022, resulting in a loss of personal dignity. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
