Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim’s (AV) care plan to perform safety checks four times a shift, because s/he was unable to locate AV’s door key. The facility failed to ensure all staff had access to AV’s room to perform safety checks as care planned. The failures resulted in the (AV) experiencing a fall, receiving a head laceration, and was transported to the hospital for stitches, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00454 $375.00 fine assessed
