Findings: The Alleged Victim (AV) relies on the facility for his or her safety. On or about July 19, 2022, around 10:40am AV had been found standing outside of the facility at the main entrance doors to the building. According to documentation the courtyard gate was unlatched. The facility failed to provide a safe environment by not ensuring the gate was secured putting AV in risk of harm, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
