Findings: Alleged Victim (AV) has a history of exit seeking behaviors. On or about July 28, 2022, AV was actively exit seeking and showing signs of agitation and unsafe behaviors. At around 7:45pm, AV barricaded him/herself in an empty room, broke the locking mechanism of a window, took the screen off and was able to jump out of the window. As a result, AV sustained a skin tear in the incident. AV was found near an intersection at the end of the street. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
