Findings: On or about July 29, 2022, Alleged Perpetrator #2 (AP2) gave the Alleged Victim (AV) two doses of his/her pain medication instead of one. AP2 did not receive proper training or oversight from the facility regarding medication administration. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
