Findings: Witness 1 (W1) has a history of aggressive behaviors and a history of altercations with Alleged Victim (AV). on or about July 29, 2022, W1 was having aggressive behaviors and striking at staff. W1 had also grabbed AV and caused a skin tear to AV, leaving a gash/abrasion to the right side, back of the hand to forearm. The facility failed to provide a safe environment to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
