- Findings
- AV and Witness 1 (W1) have a history of resident-to-resident altercations. On or about July 29, 2022, W1 struck AV several times on the side of AV's head, resulting in unreasonable discomfort. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
