Findings: The facility failed to provide appropriate services according to resident needs, relating to diabetic medication. According to an investigation, in June 2022, the Alleged Victim's (AV) blood sugar readings were unstable, with readings over 500 twenty-one times, leaving AV at risk for serious harm. It was also discovered that from approximately July 18, 2022, to July 27, 2022, AV’s blood sugar was not tested due to lack of test strips. These failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
