Findings: Alleged Victim (AV) relies on the facility for his/her care needs. On or about July 30, 2022, AV was found with a cut/skin tear to his/her elbow. AV previously had skin tears or bruising on May 16th, June 18th, July 6th, 16th and 21st, 2022. There is no documentation in chart notes or incident reports of any new or different interventions being implemented in June or July to prevent future skin issues. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of skin injuries, which is a violation of resident rights, is neglect of care and constitutes abuse.
