Findings: On or about August 2, 2022, the Alleged Victim (AV) was found in the stairway of the facility with a bruise on his/her hip. AV was last seen on August 1, 2022 at approximately 3:30 pm. When staff couldn't find AV, they assumed AV had left with family and forgot to sign out. On or about August 2, 2022 at approximately 12:30 PM, AV's family inquired to where AV was and staff launched a search and found AV in the stairwell. AV was missing approximately 21 hours. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect f care and constitutes abuse.
Sanction: ALFCP22-00937 $375.00 fine assessed
