Findings: The Alleged Victim (AV) is care planned to be toilet checked every two hours on night shift. Alleged Perpetrator #2 (AP2) was scheduled to work night shift on August 2, 2022. Staff arrived at the facility at 5:00 am and found AV and his/her bedding soaked in urine. According to video surveillance, AP2 did not check on AV during the entire shift. AP2 failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure is a violation of Oregon Administrative Rules.
