Findings
Alleged Victim (AV) relies on the facility for his/her care. AV was using a “spare” wheelchair from the facility that did not fit his/her size and staff would leave AV’s feet flat on the floor instead of on the chair’s footrest. AV would “scoot” him/herself around the facility in the wheelchair resulting in injuries to AV’s leg due to one leg being pulled underneath. On or about July 26, 2022, staff notes in facility documents that AV has three (3) bruises and a scab on his/her rights foot and that AV’s right foot is “always getting caught” under his/her wheelchair; staff also found a scabbed wound on top of AV’s foot and AV would “grunt” when staff touched his/her foot. AV has a pressure alarm that will alert staff if he/she attempts to self-transfer. AV’s chair/bed alarm was on occasions broken or staff wouldn’t move it from wheelchair to bed or from bed to wheelchair, causing AV to fall several times a day. The facility failed to provide service to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
