Findings: The facility failed to implement interventions or appropriately care plan for the Alleged Victim’s (AV) and Witness #1’s (W1) known behaviors and previous altercations with each other. This failure resulted in a physical altercation on or about August 3, 2022, between AV and W1, where both AV and W1 sustained injuries. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
