Findings: The Alleged Victim was known to need a higher level of care than what the facility could provide. The facility started the move out process for AV, but did not follow up with required documentation so the move out notice was denied. On or about August 2, 2022, AV fell at the facility and broke both of his/her legs. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse
