Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs. AV is care planned for rotation and shift of weight while in chair and bed to prevent further skin breakdown, scheduled and stand-by assist with toileting, and requires assistance with meals. On several occasions, AV was noted to be left in h/h recliner for long periods of time and soiled, which resulted in pressure ulcers. AV went without two meals due to inability to independently feed h/h self. The facility’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
