- The facility failed to implement interventions or appropriately care plan for the Alleged Victim’s (AV) and Witness #2’s (W2) known behaviors and previous altercations with other people. This failure resulted in a physical altercation on or about August 7, 2022, where AV and W2 slapped each other. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
