Findings: Alleged Victim (AV) relies on the facility for a safe environment. AV has a history of falls and has experienced approximately eight injury and non-injury falls between June 08, 2022, and August 06, 2022. On or about August 06, 2022, AV fell and was found on the floor, resulting in skin tears to elbow and wrist. The facility failed to adjust the service plan and implement reasonable interventions to address AV’s increasing and ongoing falls, and provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
