Findings: Alleged Victim (AV) was diagnosed with an infection of the urinary system that contributed to four falls between, August 03, 2022, to August 07, 2022. AV experienced pain, a cut to h/h head, skin tears, and five fractured ribs. The facility failed to appropriately adjust AVs care plan and implement reasonable person-centered interventions to address AVs ongoing falls, which is a violation of resident’s rights is neglect of care and constitutes abuse.
Sanction: RCFCP22-01829 $1500.00 fine assessed
