Findings: Witness #2 (W2) has a history of agitation and aggression towards other residents. On or about August 4, 2022, W2 had aggressive behavior towards another resident, which lead to a resident-to-resident altercation. W2 grabbed the Alleged Victim (AV) by the neck and began choking him/her, placing AV at risk for serious harm. The facility failed to implement interventions or appropriately care plan for W2’s known behaviors and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
