Findings: On or about August 11 ,2022, AV eloped from the facility and was found by Witness 1 (W1). Camera show staff left one of the back doors propped open during the food delivery when AV left the facility. AV’s service plan has AV noted for wandering and history of exit seeking. The facility failed to provide a safe environment, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01831 $250.00 fine assessed
