Findings: The Alleged Victim (AV) relies on the facility for his/her care and is known to have physical/aggressive behavior. On or about August 10, 2022, while providing care to AV, untrained staff held AV’s hands down to complete personal care. The facility failed to train staff and ensure care plans contained person-centered interventions to address AV’s behaviors, causing AV loss of personal dignity, which is a violation of resident rights, is neglect of care, and constitutes abuse.
