Findings: The Alleged Victim (AV) relies on the facility for his/her care and is known to have physical/aggressive behavior. On or about August 10, 2022, the Alleged Perpetrator 2 (AP2) made an inappropriate verbal threat toward the Alleged Victim (AV) in his/her presence and in front of two witnesses. AP2’s actions are considered verbal abuse. The facility failed to train staff, ensure care plans contained person-centered interventions and protect AV from verbal abuse, which are violations of resident rights, is neglect of care and constitutes abuse.
