Findings: The Alleged Victim (AV) relies on the facility for his/her care and is known to have physical/aggressive behavior. On or about August 10, 2022, while providing care to AV, untrained staff swatted at AV when s/he became resistant to care. The facility failed to train staff and ensure care plans contained person-centered interventions to address AV’s behaviors, causing AV loss of personal dignity, which is a violation of resident rights, is neglect of care, and constitutes abuse.
