Findings: The facility failed to Provide a safe environment for The Alleged Victim (AV). On or about August 17, 2022, AV went outside on the courtyard, and was checked on every twenty (20) minutes. At the second check AV was warm to the touch and unresponsive. Resulting in AV having to go to the emergency room for treatment for dehydration. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
