Findings: The Alleged Victim (AV) has a history of falls. On July 20, 2022, AV was identified as needing more assistance with mobility and transfers, however, AV's care plan was not updated to include AV's change of condition. On or about July 21, 2022, AV had an unwitnessed fall where he/she suffered a subdural hematoma and was hospitalized. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00244 $500.00 fine assessed
