Findings: The Alleged Victim relies on the facility for his or her safety. On or about August 19, 2022, it was discovered AV had eloped from the facility through a side door. Law enforcement agency (LEA) was contacted, and AV was found approximately thirty (30) minutes later across the street. LEA walked AV back to the facility. The facility failed to provide a safe environment and failed to protect resident from leaving the facility without assistance, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
