Findings: Witness #1’s (W1) has a history of agitation, combative behaviors, and resident-to-resident altercations. On or about August 17, 2022, W1 slapped Alleged Victim (AV) on the face and tried to grab AV walker. W1 is care planned to be redirected and to keep W1 away from other residents in common arear. The facility only redirected W1 but failed to keep W1 away from other residents in common area. The facility failed to follow care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01720 $500.00 fine assessed
