Findings: The Alleged Victim (AV) has a history of eloping the facility. On or about August 20, 2022, AV was found outside of the secured unit, in the lobby. AV left the secure unit, unnoticed by staff. It is unclear how AV exited the secure unit. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
