Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)’s needs, resulting in AVs loss of dignity. Documentation provided to the department shows AV had a Move-in Assessment, but no care plan. AV was found twice (2) in soiled brief for an unknown amount of time. AV call light was pushed for assistance and AV waited for 34 minutes and 44 minutes. Approximately several days of soiled laundry was found in AV shower, and trash was not emptied for approximately three (3) days. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
