Findings: Alleged Victim (AV) has a history of resident-to-resident altercations. AV is serviced planned for redirection when AV is within four (4) feet of other residents. On or about August 05, 2022, and August 27, 2022, AVs service plan was not being followed which resulted in non-injury resident-to-resident altercations. The facility failed to follow AVs care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
