Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to be checked every 2 hours for incontinence care. AV has been found on more than one occasion sitting in urine-soaked clothing, causing unreasonable discomfort. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
