Findings: The facility failed to follow the Alleged Victim’s (AV) care plan related to fall history. AV’s care plan states AV needs assistance to and from dining room/and or community activities. On or about August 07, 2022, AV fell walking back to h/h room without being escorted by staff after dinner. AV sustained an injury to h/h forehead and was transported to the Emergency Department. The failure resulted in AV experiencing pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00348 $250.00 fine assessed
