Findings: On or about August 31, 2022 at approximately 8:55 am, Alleged Perpetrator 2 (AP2) gave Alleged Victim (AV) another resident's medications. AV was anxious and vomited later in the day. AP2 was provided training by the facility. The facility notified AVs PCP of the medication error on August 31, 2022. The allegation that AP2 failed to provide a safe medication administration system and administer medications to AV as ordered resulting in a serious risk of harm was investigated and the determination was not substantiated. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
