Findings: The Alleged Victim (AV) is a known fall risk and will try to get up from his/her wheelchair and needs extensive assistance from staff with ambulation. On or about September 1, 2022, AV suffered two falls, one in the morning and one in the afternoon. Only one staff was working in the cottage where AV resides. AV did not have an assessment after his/her fall, as there was no other staff to complete the assessment. AV fell again on September 15, 2022 when AV's one to one caregiver wasn't at work. The facility failed to ensure there were enough staff to ensure AV's safety, and also failed to put interventions in place after AV's earlier falls to prevent further falls. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
