Findings: Alleged Victim (AV) had a known history of cognitive decline and an inability to find their way home. Documentation shows that on or about August 19, 2022, AV was observed multiple times trying to leave the facility in the middle of the night. There were no additional interventions put in place to ensure AV is safe and doesn’t leave the facility. On or about August 19, 2022, AV left the facility, and was found blocks away after riding the Max train, putting AV at risk for serious harm. The facility failed to care plan regarding AV's known behavior of wandering, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP23-00323 $500.00 fine assessed
