Findings: On or about August 31, 2022, the Alleged Victim (AV) and Witness 1(W1) were involved in a resident-to-resident altercation. The facility failed to follow W1’s care plan that stated to redirect residents that get into W1’s space. The failure resulted in AV being slapped by W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
