Findings: Alleged Victim (AV) is a two-person transfer. On or about September 3, 2022, Alleged Perpetrator 2 (AP2) used a sit to stand and did a one-person transfer. As a result, AV’s leg got crushed in the process resulting in bruising on his/her leg and a skin tear. AP2 failed to follow the care plan, which is neglect of care and constitutes abuse. The facility failed to assure AV’s care plan was being followed, which is a violation of Oregon Administrative Rules.
