Findings: The Alleged Victim (AV) is care planned for assistance with toileting/ brief changing. AV is noted on his/her care plan to be exceptionally difficult with toileting / brief changes. On or about September 7, 2022, Alleged Perpetrator #2, #3 and #4 all participated in physically forcing AV from the common area to his/her room to have his/her brief changed. Once in AV's room, AV's hands were held down to prevent AV from hitting staff. AP2 and AP4's actions are a violation of resident rights, are considered neglect of care and constitute physical abuse. The facility's failure to protect AV from physical abuse is a violation of resident rights, is considered neglect of care and constitutes abuse.
