Findings: The Alleged Victim (AV) is care planned for assistance with transfers and assistance with toileting. On or about September 6, 2022, Staff assisted AV with changing his/her depends by lifting his/her legs into the air to change him/her. During this action, AV expressed pain. AV's care plan does not direct caregivers on how to properly change AV and different staff gave different accounts of how to properly change AV. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
