Findings: On or about September 5, 2022, Alleged Perpetrator #2 (AP2) changed the Alleged Victim's (AV) brief in the common area of the facility. Other residents and/or staff were known to be in the common area during this time and AV was indecently exposed to others, causing a loss of dignity to AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to protect AV from a loss of dignity is a violation of resident rights, is considered neglect of care and constitutes abuse.
