Findings: Alleged Victim (AV) has a history of falls. AV is care planned for nonslip socks, regular safety check and to encourage AV to use h/h walker. AV has had five (5) falls approximately between February 18, 2022, to August 08, 2022. On or about August 08, 2022, it was discovered AV had a rug burn on h/h left knee, bruising on left arm and a bloody nose. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00107 $500.00 fine assessed
