Findings: Alleged Victim (AV) has a history of multiple falls with injuries. On or about September 04, 2022, AV was found on the floor resulting in a large bruise to the elbow. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
